Why are textures appearing as blocks in Nioh 2 levels?

Entering complex levels felt anxious as distant textures emerged like blocks due to massive loading lag. I tried using the built-in Windows disk defragmenter, but that's useless for NVMe SSDs and just made my system feel more sluggish. I used the GamePP NVMe Queue Depth Configuration to increase the depth from 32 to 128 and optimized the concurrent read mode. CrystalDiskMark showed 4K random reads improved to 62-75MB/s, and HWiNFO confirmed full-load temperatures stayed between 52-60℃.
